When nobody on site knows how to investigate
No safety person, a safety person who just left, or one too busy to run it. We step in and handle the process.
Three steps, no meetings
Answers your leadership can act on
Informed by the most trusted industry standards
Every report keeps three things apart: what was observed, what we recommend, and what a regulation actually requires. Nothing is sent until SafetyGrid Safety Services has reviewed and approved it.
Your incident desk, every month
Hand us your full investigation process. We investigate every incident you send and give you a monthly report on where the problems are and what to fix first, starting in month one. Your crews run the site. We handle the investigations and the paperwork.
- A monthly report built from your own submissions: near misses, first aid cases, injuries and property damage
- Top problem areas and recommended actions, ranked by the hierarchy of controls
- Quiet month? An industry and seasonal outlook, clearly labeled as general guidance
- Unused investigations roll over for up to 3 months while your plan is active
- Reports with your company's name on them
- A training package for your team: short leadership and crew videos, a job board poster with QR codes, and a toolbox talk card
- Reports written in your company's style, from a blank template or a sample with confidential details removed
- Extra investigations $250 each
How company style matching worksSend a blank template or a sample with confidential details removed.ReadClose
Every company writes incident reports its own way. On every Incident Desk plan, we match yours. Send us a blank report template, or one sample report with all names and confidential details removed. We don't need your actual reports or any sensitive information, only how your reports are written.
The investigation itself stays to the SafetyGrid standard. Every report includes the root cause and corrective actions, and is approved by a qualified safety professional.

Investigations are desk reviews based on the information you provide. Reports are informed by industry standards and are not a certification of regulatory compliance. Incident Desk reports and recommendations are advisory; your team decides what to act on and keeps its own OSHA records. Unused plan investigations expire 3 months after the month they were included in, or when a plan ends. The single investigation credit applies to one single investigation fee per company, toward the first month of a new Incident Desk plan started within 30 days of that report.
